Inner City Assertive Outreach and Case Coordination for Homeless Vulnerable and Complex Clients

This service (operating as Way2Home) supports men and women who are experiencing chronical homelessness and have complex needs, particularly those sleeping rough in the Sydney local government areas. It assists clients to transition from sleeping rough or living in improvised dwellings into long-term accommodation and supports them to sustain long-term housing.

The program works closely with other housing and health services in order to assist clients access safe and affordable housing, to address their health needs and to re-engage with their communities. It also engages with clients to support them with required documentation for housing applications.

The service has a strong focus on responses for Aboriginal people from culturally and linguistically diverse backgrounds.
