Murrumbidgee Homeless Youth Assistance Program (HYAP)
This service operating as Reconnecting Home, facilitates urgent or prompt brokerage to address the individualised needs of children and young people ages 12 – 15 years old who are homeless or at risk of homelessness in the Murrumbidgee area.
Brokerage services are used to deliver safe and supported accommodation, case coordination and access to therapeutic supports. Health services are also provided such as mental health services and drug and alcohol services.
The service also provides advocacy and quality assurance to ensure case plans and services are keeping the child safe and reconnecting the young person with their key support relationships, accommodation, health, wellbeing and education.
This service has a strong focus on Aboriginal young people and people from culturally and linguistically diverse backgrounds.
