Western NSW Homeless Youth Assistance (Upper Western)

This service, operating as HYAP – Upper Western NSW, supports children and young people ages 12 to 15 years old who are experiencing homelessness or at risk of homelessness.

This service is designed to promote the safety and wellbeing of children and young people through strengths-based, client-centred responses, trauma-informed practice, wraparound support, and promoting continuity of care and culturally responsive practice.

Coordinated and intensive case management support is delivered, working alongside other specialist services, e.g. health, drug and alcohol, juvenile justice and child protection. A range of programs to support young people is also offered, including educational and personal development; cultural engagement; living skills; drug and alcohol programs; and family-focused programs.

Wraparound support for the young person’s family will also be provided, where appropriate.

For clients with complex needs, the service provides intensive and integrated support through a multi-disciplinary model working with specialist services, e.g. mental health, trauma counselling, and drug and alcohol services.
