This online form is to be used for Homes NSW and AHO tenants wanting to advise changes to their tenancy. This form can also be used when we have written to you in relation to a rent subsidy review we are undertaking for your household. The tenancy online form can be customised, allowing for multiple forms to be completed at once, without having to repeat the same details.

Forms that can be completed as part of tenancy online are:

  • Application for Additional Occupants
  • Authority for Rental Deduction
  • Income Confirmation Scheme
  • Details of Property and Land Ownership
  • Medical Assessment
  • Independent Living Skills Assessment
  • Notice to Vacate.

  • Centrelink Reference Number
  • Australian drivers licence, Australian passport or Medicare card
  • documents supporting your request.

Email address and contact number of healthcare professionals/support providers if you are including a Medical Assessment and/or Independent Living Skills Assessment form.
