Consent forms required by Homes NSW
To minimise delays during the Home Modifications (Disability) process, we require a completed and signed Consent form to allow Homes NSW to discuss and exchange information about the client’s modification needs with a third party, or their agency support worker.

An Occupational Therapist Summary Report is required to accompany your occupational therapist report
When an occupational therapist report is required for the requested modifications, it must be accompanied by the Occupational Therapist Summary Report.
This Summary Report requests an occupational therapist to immediately report any health and safety requests to the Maintenance Hub and provides Homes NSW, in summary, the identified modifications required for the client to access and live safely in their home on an ongoing basis.
Joint Onsite Inspections of Homes NSW managed properties
When a Homes NSW managed property is offered for a potential tenancy to a client that requires Major Modifications, in most cases you will be required to attend an initial inspection of the property, jointly with the client, and Homes NSW staff.
This allows clients and their occupational therapists to determine the suitability of the property to meet the medical and disability needs of the client, and for the occupational therapist and staff of Homes NSW to determine whether the property can structurally accommodate the required modifications. The joint inspection can also determine any specific details needed for the occupational therapist report and ensure the Occupational Therapist Summary Report is provided.
What we need from your clinical expertise
Your clinical expertise will advise Homes NSW of the medical or disability barrier to the client living safely in the home, the modifications that can assist the client, and the criticality of those modifications in regard to the client’s health and safety.
Ultimately, these will be clearly listed in the Occupational Therapist Summary Report.
